New Patient Health History Form - Required

In order to be compliant with all the new regulations and to make things as time efficient and smooth as possible, we have upgraded our office systems.  Please click the following link to go to our patient intake form.  You do not have to print them; just fill them out COMPLETELY.   This will save you  time in office, and will insure we have all of your current information.  Our new systems offer a variety of services, which will make things much easier both for you as our patient and for the office.  This new system, makes reminders, statements, chart notes, and documentation easier and more complete. It also gives us a powerful tool when dealing with your insurance company.  So please take a few minutes to fill this out.
